The cause might be a lump in the small intestine which leads to inflamation of the intestine This must be treated with antibiotics as soon as possible. If not treated then symptoms gets worsens.

Enlarged small intestine is a wrong word and i think your uncle might have appendisitis which is located at the junction of small and large intestine. Infection of appendix can cause acute pain,vomiting and may require emergency operation
