A post-op transexual is a person who has had his or her body surgically changed to the other sex. A transexual is someone whose psychological gender doesn't match the physical body they were born with. In some cases the person may have their body medically altered to match the gender they feel they really are.

A man who is a post-op transsexual has an XX chromosome pair, and was born with ovaries. Likewise, a woman who is a post-op transsexual has an XY chromosome pair, and was born with testes. After undergoing sexual reassignment surgery, their bodies are now the gender with which they identify.
