The best answer is to consult an APA guidelines website. In summmary, the title of the article, date, author, instructor if submitting to a college, and course information are included. Depending on the type of paper being submitted, certain information also goes in the header, but normally this is just an extension of the title.

In the upper half goes the paper's title, the author's first and last names and middle initial (no personal titles like "Dr." or degrees like "M.A."), and any institutional affiliation--all centered and double spaced. The title goes on top flushed left in ALL CAPS after the legend "Running head:" and the page number "1" goes in the upper right corner.
