I say "Bless You" when someone sneezes, but when someone coughs I ask "are you okay?" I suppose "Bless you would work too, and it is very caring.

"Bless you" is traditional for sneezes, but there is no equivalent for a cough. If it was very bad, it would be kind to ask if they're OK.

I usually just let them cough and then ask them if they are ok or if they need a cup of water!

When someone coughs, I usually don't say anything. I reserve my "bless yous" for sneezing. But I rarely will say "bless you" even then, because I figure that it is assumed that a person is blessed. Sneezing or coughing alone is not reason enough for a person to be blessed, unless it is. And then I figure they are.
