There was no "real Billie Jean". Billie Jean was more so an idea that Michael Jackson came up with after dealing with groupies for so long. Billie Jean is just meant to represent the groupies that hung around backstage and so forth, not one specific person.

Michael Jackson has said that he had no specific woman in mind for "Billie Jean" but rather the song is based on the various groupies that followed the Jackson 5.
