Before you write the letter, consider whether your fall really was the fault of the retailer, or if the fault was your own. If it was their fault, you should include information about how you fell and why, where, and when. Describe the extent of your injuries, and what type of compensation you desire.

Your insurance company will contact them to pay for any medical treatment you received. If you are looking for a monetary settlement, you would do well to engage the services of an attorney.

You should provide a clear description of the event along with the names of any witnesses. You should explain why you believe they are liable to you. You should set out exactly what you expect from the retailer. You need to provide them with copies of your medical bills

Before you write a demand letter make sure you have proof, witnesses and you reported it before you left the store. Also have a doctor's note and hospitals bills.

Address your letter to the manager by name. Clearly and concisely detail when and where your fall took place; what happened after your fall; a verifiable account of your injuries; and a clear request for what you want the store to do.
