Depending on how often you put miles on the car. Typically recommended one flush every 30,000 miles and change once every 3 oil changes.

You have to take it into the garage and they will take the tranny out and pull the screen if it is dirty they will replace it then it should be ok if not then you have to replace the transmission or it could be that your tranny has bad band that also means replace.

A good rule of thumb is flush 3 times change once. A flush is almost as good as a change in most situations, but it may cause "gunking"

If you need a new transmission entirely, you will feel your car "slipping" when changing gears. You can also check the color of your transmission fluid (should be bright red).

I'll assume you mean a transmission FLUID change vs. flush. Basically, a flush is a better change, they remove all of the old fluid, rather than just draining what will fall out on it's own. I recommend always flushing an auttomatic transmission, rather than just a drain and replace.

Usually the manufacturer will give a recommended flush interval. If the car is driven hard then it may need to be changed more often. Flushing helps to get rid of older fluid that doesn't typically get removed during a fluid change so it's not needed as frequently.
