The easiest way would be to get some real sturdy 2x4's and plywood. You can build a frame that is at a safe angle, and wide enough to accommodate the chair, then measure and cut the plywood to fit over top. Place the frame where needed, and attach plywood.

I had a friend who had a disabled mom and all the did was stack up concrete slabs and use a few layers of wooden boards. It worked well and was cheap.

You have no choice when installing a ramp, major modifications will be needed. You need to connect the ramp to the part of the house under the doorway which will require a lot of nails, this ramp will most likely be permanent and will remain there for a long time.
