You must include your name, address and social security number, the account number and evidence to support your claim (cancelled checks, letters to the creditor, bills showing a zero balance). If the debt is not yours, explain that the creditor is unable to provide proof of the debt. Be succinct and thorough. Keep a dated copy.

You will want to send a letter with a clear explaination of the disputed item along with copies of documentation verifying your situation. In the letter request that an action be taken regarding the complaint. Send the letter via certified mail and request a return receipt. Wait approximately 30 days.

let them know your name, social security number, the account that you are disputing and what you are disputing, and why you are disputing this.

The first thing you need to do is to contact the agency and informing them of the dispute. If you get no satisfaction that way have them send you out a formal dispute form. State your claim clearly and attach any supporting documentation you may have. Give them thirty days and if no answer send another letter, registered.
