The treaty for extradition between Italy and the USA was signed in Rome on October 13, 1983. Requests for extradition are made through the Italian Justice Ministry or the USDOJ.

Yes, the United States and Italy do have an extradition treaty. It was signed on October 13, 1983. It went into effect on September 24, 1984. People have been extradited from Italy on a regular basis since it went into effect.
