You should have the parents of your grandchild write out a note that states that you have the authority to make medical decisions for the child.

If you are within this country and are traveling more than a short time, a temporary power of attorney by the parents is a good idea. Thus you could take the child to the doctor while traveling. Have the child's ID paperwork or ID forms, if they exist.
